Self Pay

A $140 office visit fee is due at check-in for all self-pay patients. This fee covers the provider evaluation and examination.

Additional charges may apply for laboratory testing, procedures, medications, injections, X-rays, supplies, or other services performed during your visit. After your visit, please return to the front desk to complete payment for any additional services received.

Payment is required in full at the time of service.
